Scotland to invest additional £10m on telehealth

The Scottish Health Secretary, Alex Neil, has announced the investment of an additional £10 million [grow_thumb image=”https://telecareaware.com/wp-content/uploads/2014/03/Healthier-Scotland.jpg” thumb_width=”150″ /](about US $ 15 million) to support home health monitoring solutions across Scotland, the BBC has reported. This additional funding for the NHS Boards will enable people to use technology such as tablet computers and smartphones to monitor conditions such as diabetes, heart problems and lung problems at home, the report added.

In 2011, Nicola Sturgeon MSP, Cabinet Secretary for Health, Wellbeing and Cities Strategy in Scotland set out her strategic vision for achieving sustainable quality in the delivery of healthcare services across Scotland, in the face of the significant challenges of Scotland’s public health record, changing demography and the economic environment. The Scottish Government’s 2020 Vision is that by 2020 everyone is able to live longer healthier lives at home, or in a homely setting and, that Scotland will have a healthcare system where, amongst other goals, there will be a focus on ensuring that people get back into their home or community environment as soon as appropriate, with minimal risk of re-admission.

Mr Neil’s announcement was made during a debate in the Scottish Parliament entitled ‘Update on Delivering the 2020 Vision in NHS Scotland’. Mr Neil claims past efforts in telecare and telemedicine have meant that £2.8 million has been “ploughed back” into Scottish frontline services and reduced emergency admissions to hospital by 70 per cent.

Attitudes and behaviours of apps users

Health apps are often in the news these days. Ofcom, the UK telecommunications regulator, commissioned Kantar Media to carry out qualitative research on attitudes and behaviours of apps users when using devices such as smart phones and tablets. The resulting report, Apps Environment, published today by Ofcom makes interesting reading.

Although the report was commissioned by the UK regulator, I think the findings are applicable to any country. What’s more, the generic nature of the conclusions make them relevant to health apps as much as to any others.

The report says that “among app users, several perceptions were found to be influencing behaviours, attitudes and the low level of concern in the app environment. These perceptions were:
• official app stores monitor and vet the apps they make available;
• well-known brands provide a safe, secure and reliable user experience, and;
• apps are safer than browsers due to their more limited and contained nature compared to browser-based internet use.”

Another finding was that many app users said they paid very little attention to permissions requested by apps – nothing new there then. How many times have you ticked that “I Agree” box to the terms and conditions without reading them?

Very few participants in the research are reported to have raised spontaneous concerns about apps. The researchers say that when prompted, in-app purchasing and advertising were of greatest concern to parents, and at most, a frustration or annoyance to others.

This shows that when it comes to more critical apps such as health apps or those that are designed to provide a safety net in a care environment a stronger regulation may be needed as well as a pro-active educational effort to emphasise the need to be aware of issues such as data privacy.

Another ‘bionic pancreas’ in test

Another possible weapon against the Continuing Battle of Stalingrad faced by diabetics is in test in the Boston area. A system developed by associate professor of biomedical engineering at Boston University Dr. Edward Damiano (whose son has Type 1 diabetes), and assistant professor at Harvard Medical School Dr. Steven Russell has a sensor inserted under the skin that relays hormone level data to a monitoring device which sends data to an app on the user’s smartphone every five minutes. The app calculates required dosages of insulin or glucagon to maintain optimal blood sugar levels, and communicates the information to two corresponding hormone infusion pumps worn by the patient. Their target for FDA approval and rollout is 2017. Wearables more than trackers…family communicators

[grow_thumb image=”https://telecareaware.com/wp-content/uploads/2013/12/Zoomed.jpg” thumb_width=”180″ /]This article from Connected World, despite the title of Will your kid wear wearables?, is a look at Revolutionary Tracker, which has developed two products from a GPS-enabled tracker to a simplified smartwatch. Both read to a smartphone for GPS tracking and communication. Where it differs is that the company broadly, not narrowly targets, ‘family tracking and communications’ as a modish wearable–infants, children, special needs children and adults (the autism market which most trackers have concentrated on), older adults and pets. Lone workers are another market, and a newer market: groups such as in camps, school trips and residential communities. It is also unusually made in USA, and the founders already have in the works a more sophisticated-looking design with multiple buttons and text functionality.

Temperature sensing chip implanted in man’s arm

A biohacker called Tim Cannon, has had a computer embedded in his forearm, to allow his bodily data to be monitored. The device called Circadia 1.0 was built by Tim and colleagues from Grindhouse Wetware. In this first version, the chip records body temperature and transfers it in real time via Bluetooth to any Android-powered mobile device. Three LEDs built into the package serve as status lights (which glow visibly under the skin). The device’s battery charges wirelessly.

Because a regular surgeon wouldn’t be allowed to carry out the implant as it is unapproved by medical authorities, Tim relied on the expertise of body modification enthusiasts to implant the device! …Yes, it’s a bit crude, but more likely than not is another small step towards our cyborg future!

The lack of evidence in health IT and patient engagement

In health economist/consultant Jane Sarasohn-Kahn’s lengthy analysis of the IMS Research report, Patient Apps for Improved Healthcare: From Novelty to Mainstream, ‘mainstream’ does not necessarily mean that apps deliver value–in health outcomes, health support or behavior change–which is why doctors have largely ignored them. For the 43,000+ ‘health apps’ so categorized in the Apple iTunes store, only 23,000 met IMS’ criteria of a ‘genuine health app.’ Few apps manage chronic disease for the highest health spenders or assist seniors, amazingly 5 apps =15 percent of all downloads with most apps having less than 500 downloads. Most apps provide information only and only 20 percent capture/track user data. Not dissimilar to the Manhattan Research smartphone study [TTA 30 Oct], the bulk of apps address behavioral health, eyes and hearing, endocrine and nutrition, heart/circulatory, musculoskeletal, and cancer. Patient groups most likely to use mobile phones for health: study

A new Manhattan Research study, which starts off rather pedestrian, contains a surprise. The trend of increased used of smartphones for health is not it, as it follows online use: 95 million Americans now use their mobile phones for health information or tools, up 27 percent from 75 million a year ago, and the Cybercitizen Health US 2013 study of over 8,600 Americans found that 38 percent of online smartphone users consider it “essential”. What sets the study apart from the usual enumeration and semi-puffery is the discovery of which patient groups find smartphones most useful and are most likely to use their phones for health reasons. Turn up, tune in but don’t drop out with health monitoring earphones

microsoft_septimu_earphonesAs part of a recent research project, Microsoft has incorporated health and fitness monitoring into a pair of earphones.

One application being developed for the hardware platform named Septimu, is a smartphone app called Musical Heart. The app enables Septimu to generate tunes based on a person’s mood or activity. So for example, fitness enthusiasts who want to keep the heart rate high can use Musical Heart to automatically up the tempo, helping them keep up the pace. Or for those feeling stressed or angry, Musical Heart could select something more soothing to help bring the heart rate and breathing down to a more relaxed level. Reported in PSFK

A timely study published online last week in Proceedings of the National Academy of Sciences, demonstrated that ‘musical agency’ (i.e. music chosen by the study participants) greatly decreased perceived exertion during strenuous activity. (more…)

Relax, it’s (not) just a breathing monitor headset

breath monitor treadmillA headset which monitors the quality of your breathing is being developed by BreathResearch, a San Francisco Bay area start-up.

Combined with a mobile app, the ‘Breath Acoustics’ headset listens to your breathing and analyzes the patterns. Sensor-based breath monitoring may be a less commonly monitored biometric pattern but recent studies suggest it could be used to detect stress levels, bacterial infections and other conditions. The headset also presents other biometric data, including heart rate, pulse oximetry, and respiration.  (more…)

Owlet baby monitor sock moving to market

It definitely ‘socked it” to this Editor at the time as an elegant way to monitor baby vital signs or signs of distress; it is a sensor-rich sock transmitting to a smartphone. While it still had pending patents, needed further prototyping and of course no FDA approval, I recommended “if you’re an angel looking for a highly marketable telehealth item–and with adult uses in hospitals and nursing homes–a trip to Utah may be in order.” Owlet is now seeking $100,000 in crowdfunding directly via its website and Amazon checkout (it was rejected by Kickstarter for being a baby product!) and will ship the device at an early bird rate of $159 once the funding is achieved. Addicted to mobile health? Telepsychiatry to the rescue!

Dr. Joseph Kvedar of Boston’s Center for Connected Health counted himself in this Editor’s camp as annoyed by the mHealth hype (inflicted by those we’ve dubbed the D3HDigital Health Hypester Horde), and far more of a believer in SMS for health programs. His blog post is a ‘kind of edge’ towards thinking that mHealth can be habit-forming. In the CCH’s own clinical trials, more participants have smartphones (tracking the general population’s adoption) even with the lag among those with chronic disease (maybe a question of affordability?) and want apps. And then he sees the pattern of people checking their smartphone obsessively, like budgies with bells and mirrors….along with a study that indicates that patients with a passive sensor to upload blood glucose measurement, rather than pushing a button, were “significantly more adherent to their plan and had better health outcomes.” Not having to do something in the Diabetic’s Perpetual Battle of Stalingrad is addictive? Gestational Diabetes Telehealth trial at John Radcliffe Hospital, Oxford (UK)

[grow_thumb image=”https://telecareaware.com/wp-content/uploads/2013/07/John-Radcliffe-Hospital.jpg” thumb_width=”150″ /]Diabetes in pregnant women, known as gestational diabetes, is said to have significantly increased over the past 20 years and affects 1 in 20 pregnant women in the UK. This  is probably caused by an excess intake of carbohydrates, says Dr Lucy Mackillop of John Radcliffe Hospital, in an interview in Inside Health on BBC Radio 4, broadcast on 23 July 2013.

A foetus growing in a high sugar environment can lead to an overweight baby resulting in birth difficulties. Such babies may also develop problems such as diabetes in later life. Gestational diabetes also carries all the usual dangers of diabetes for mother.

If a pregnant woman has one of 5 risk factors she will be fully tested for diabetes and if she is diabetic, she will be monitored during preganancy. Monitoring typically may be a fortnightly hospital visit, but at the John Radcliffe there is a trial of 50 mothers who have been given special blood sugar meters which connect to smart phones via Bluetooth.

A daily blood sugar test result is transmitted to the hospital where software picks out patients that may need attention by a midwife, and changes to the treatment regime can be implemented if necessary. This saves many unnecessary hospital visits while giving a much more frequent review of the state of the patients.

Is Saga automated lifelogging the future of memory – or just creepy?

The Saga (nothing to do with the UK company of the same name) smartphone app takes journal-keeping to an entirely new level, that is without input by you. It takes data behind the scenes from your smartphone and tries to make some sense of it. For example, you visit lots of burger bars…you must like burgers (unless you are a hygiene inspector, of course). So is it, as the developer claims in the 9 minute video below, the next evolution of memory, for which we will all become grateful or is it just creepy that people would want all that activity logged automatically? Doctors wild about…what works

In the first half of the following blog item the author makes some valid points about doctors being quick to adopt mobile devices but that they were also quick to discover that the available apps are not much use in their work. The second half turns into a ‘knock Apple and big-up Windows 8 on tablets’ session. But then, as the author is Bill Crounse, MD, Senior Director, Worldwide Health at Microsoft, it would be surprising if he didn’t take that opportunity. UPDATE: related item, thanks to Toni Bunting: Health apps won’t reach core NHS patients (The Guardian). An NHS commissioner, writing under a pseudonym, also bemoans the lack of focus on appropriate apps and/or their use in the NHS. What the author focuses on is that the majority of NHS users are “the elderly, deprived and poorly-educated” and these people are less likely than most to be wielding and using smartphones.
